With deep regret, we announce the permanent closure of our Miami location 13500 SW 88th St., Suite 175 & 171 Miami, FL. 33186, after more than 25 years of dedicated service to the community. This decision was not made lightly, and it comes with sincere gratitude for the trust and loyalty you have shown us over the years.

We understand that this change may cause inconvenience, and we truly appreciate your patience and understanding during this transition.

If you need assistance with prescription refills, access to medical records, lab order referrals, or have any other questions, please contact our Plantation location at 954-791-0711, or call 305-387-0081 and leave a message to request that your medical records be sent directly via email.

If traveling to Plantation is not feasible, we recommend contacting your health insurance provider to be reassigned to a local, in-network physician. You can do so by calling the customer service number on the back of your insurance card.

For our patients in the Miami area, we suggest Baptist Health, particularly the Baptist Primary Care – Town & Country location at 11535 SW 88th St., Miami, FL 33176. They can be reached directly at 786-595-8000. Baptist Health offers an extensive network of primary and specialty care providers and is in-network with most major insurance plans.
